Birth Certificate is a valuable document being issued by the Office of the Registrar of Birth and Death of the Government of the People’s Republic of Bangladesh. It should always be either in your own custody or in the custody of an authorized person. If lost or destroyed, the fact and circumstances should immediately be reported to the nearest Bangladesh Embassy and the local Police.
Birth Certificate is an essential document for applying for (a) Machine Readable Passport; (b) Marriage Registration; (c) Admission into educational institute; (d) Driving License; (e) Voter ID or National ID; (f) Bank account; (g) Import, Export or Trade License; (h) Utility Connections; (i) Tax Identification Number; (j) Contractor License; (k) Registration for Car etc.
Requirements for Birth Registration Certificate
Please duly fill-up the Birth Registration Form-J and send it along with the following documents:
- Passport size one color photograph with white background taken recently within the last six maonths;
- Copy of Bangladesh Passport;
- Money order/Bank Draft/Certified or Cashier Check of US$ 6:00 payable to the ‘Embassy of Bangladesh’. No personal check or cash is accepted;
- Self-addressed prepaid return envelope with tracking number
No Refunds: Processing fees once received is nonrefundable, even if the application is withdrawn or service cannot be rendered.
Service by Mail: Outstation applicants may send their applications for consular service by mail. Applicants must add return prepaid envelope with appropriate postage, using the USPS Express/Priority and Certified mail service. The Embassy will not accept any return envelope without proper postage. Payment by credit card or account payee number will not be accepted as per the policy of the Embassy.
Please note that if any mail received before 10:00 am on a working day will be deemed as received on that day. Mails received after 10:00 am on a working day will be treated as received on the following working day.
For faster service: Please apply through Online Application for birth registration certificate. After submission, please take a printout and attach it with your Birth Registration Application (Form-J) and all required documents to the Embassy.

How to Submit Online Application: On the online application, please select Bangladesh Missions from scroll-down menu; then select USA; then select Washington DC; then select Ward-1 and click on next and fill-up the first page in Bangla using ‘AVRO’ software except passport number in English and second page completely in English then submit the application.
